Lumbar Procedures

Lumbar procedures are surgical treatments directed at structural problems in the lower spine when non-surgical care is no longer sufficient or when nerve compression, instability, or progressive symptoms require intervention.

The goal is to treat the specific anatomical problem responsible for symptoms while preserving as much normal tissue, movement, and spinal function as possible.

Conditions Treated With Lumbar Surgery

Lumbar surgery may be considered for selected patients with conditions that create mechanical compression, structural failure, or persistent neurological symptoms.

  • Lumbar disc herniation with persistent nerve compression
  • Lumbar spinal stenosis causing leg pain, numbness, or walking limitation
  • Spondylolisthesis or instability when the spine cannot support normal loads
  • Recurrent or complex nerve compression after prior treatment
  • Selected cases of degenerative lumbar disease requiring decompression or stabilization

How We Choose the Right Procedure

The choice of lumbar procedure depends on the source of symptoms, the degree of nerve compression, the integrity of the disc and joints, and whether the spine remains mechanically stable.

In general, lumbar surgical options fall into three categories:

1. Targeted decompression

Used when symptoms are driven primarily by pressure on a nerve. The goal is to relieve compression while minimizing disruption to surrounding tissues.

2. Disc fragment removal

Used when a disc herniation is compressing a nerve and symptoms persist despite appropriate non-surgical treatment.

3. Stabilization or fusion

Used when decompression alone is not enough because the segment is unstable, collapsed, or structurally unable to support normal loads.

We do not select a procedure based on imaging alone. We correlate symptoms, examination findings, imaging, and functional limitations to determine which structure is responsible and which intervention is most appropriate.

When surgery is needed, we prioritize the least disruptive technique that can reliably solve the problem. A smaller procedure is preferred only when it provides a durable and complete treatment for the patient’s condition.
