Neck pain is a common and sometimes disabling condition that can result from disc degeneration, herniation, or spinal stenosis.

While many patients improve with conservative treatment, some require surgical intervention. When surgery is considered, the key decision is whether the affected segment can be treated while preserving motion, or whether stabilization is necessary.

In general, surgical options fall into three categories: motion preservation, stabilization, or targeted nerve decompression.

The main surgical interventions are cervical disc replacement, cervical discectomy and fusion, and posterior cervical foraminotomy.

How We Choose the Right Procedure

Surgical treatment of the cervical spine is not one-size-fits-all. The choice of procedure depends on which structures are involved and how they are contributing to symptoms.

In general, there are three approaches:

1. Motion Preservation (Disc Replacement)

Used when the disc is the primary structure affected, and the surrounding joints remain healthy. → Goal: treat the problem while maintaining near-normal movement

2. Stabilization (Fusion / ACDF)

Used when there is more extensive structural involvement, including: Facet joint degeneration Disc collapse Instability or hypermobility → Goal: restore stability and relieve nerve compression

3. Targeted Decompression (Endoscopic Foraminotomy with or without partial discectomy)

Used when symptoms are caused by isolated nerve compression, and: The disc retains its integrity There is minimal neck pain The motion segment remains stable → Goal: relieve nerve pressure while preserving native anatomy
